Please take a minute to fill in the following form 2 hours previously from your tattoo appointment, or within your arrival at the studio.

You acknowledge by signing this agreement that you have been given the full opportunity to ask and all questions which you might have about the obtaining of a tattoo and that all of your questions have been answered to your full satisfaction.
